Residential Programs

Price Street : West Chester
Women only

These housing locations provide permanent supportive housing for 18 year or older, single women who
Have serious and persistent mental health challenges
Previously lived on the streets
Resistant to services
The programs use a “housing first” approach with emphasis on getting individuals off the streets and into safe housing
Our staff works to build trust and engagement so that the individuals will link to services such as health care, mental health, disability services, etc.
On-site staffing is provided 

Safe Haven : West Chester
Women Only

These housing locations provide permanent supportive housing for 18 year or older, single women who

Forensic House : Matlack : West Chester
Men only
The Forensic Half Way House, also known as Matlack, is a residential program funded by HUD (Shelter + Care) and the Chester County Department of Mental Health/Intellectual and Developmental Disabilities
Program provides eight beds for adult males with severe mental illness who are homeless and involved in the Chester County Criminal Justice System
Staffed on a 24 hour basis
Provides assistance with adult daily living skills and coordinates care with case management, treatment and probation services

Liberty House : Phoenixville
This program is a supported living arrangement (SLA) that provides housing and support services to 51 adults and one family in Phoenixville
The daily supports include
Medication management
Crisis counseling
Unit inspections
Transportation
Communication with other treatment team members
Direction to appropriate community services and/or resources
Qualifying residents must meet the HUD definition of homelessness, have a mental health diagnosis, be willing to use appropriate treatment and services, and be eligible for Section 8 housing
Program consists of 28 Single Room Units, 13-one bedroom apartments, and 7-two bedroom apartments

Community Residential Rehabilitation (CRR) : West Chester
It is a licensed group home program for persons with severe mental illness who need 24 hour supervised living arrangement and support
Staffed with residential support 24/7
Individuals receive assistance with adult daily living skills
Staff also work closely with members of the individuals treatment team to coordinate care and support the residents’ recovery in the community.
