Case Management, available 24 hours a day, provides assistance to adults with serious and persistent mental illness and to children and adolescents with serious emotional disturbances and their families. The Program provides assistance in gaining access to needed mental health, medical, social, educational and other resources. Activities are designed to provide an individualized approach to serving the daily needs of individuals and families to enable them to develop their strengths and improve the quality of their lives in the community. 

There are specific Pennsylvania Department of Public Welfare eligibility requirements for the recipients of the service

Blended Case Management - Children

​The Child/Adolescent Blended Case Management Program provides case management services to children, adolescents and their families
It is a voluntary 24-hour service providing case management to children and adolescents (and their families) who have severe and persistent mental illness
Services are provided at two levels of intensity
Level I provides intensive case management contacts
Level II and RC provides an intermediate level of services when the consumer is managing well
The goal of BCM is to assist the consumer in achieving and maintaining recovery
